To Prospective Patients and Family Members:

Thank you for your interest in the Columbus Specialty Hospital.  Our evaluation and admissions process is designed to be responsive, informative and easy to negotiate.  This process begins with a referral from your physician or from a case manager in the hospital you are currently a patient at.  The referral can be called into 706-321-6712 or faxed to 706-321-6616.

Once we receive the referral our clinical liaison nurse will review the information to determine if you can benefit from our program.  If it appears that you might benefit from our program our clinical liaison nurse will travel to your location and evaluate you for possible admission.  If the admission criteria have been met for our services we will coordinate with your insurance carrier and, if approved for admission, work with staff at your hospital to arrange for transport to Columbus Specialty Hospital.  We are also able to admit directly from your physician’s office or from your home if our admission criteria have been met.

Our hospital is a long-term acute care hospital where the average length of stay is twenty-five days or longer.  While a patient here your physician may admit and follow you, or one of our physicians can follow you.  While a patient here our medical team will keep you informed of your program goals as well as your progress toward those goals.  Your family will be encouraged to visit and also remain informed.  Our case managers will provide ongoing information to your insurance carrier, answer questions for you and your family members, and develop a discharge plan with you that best address your needs following discharge from our hospital.
